Why should there be an AIDS confidentiality principle?

If AIDS is detected during the premarital examination, why can't the doctor tell the spouse? If the sick party conceals it, isn't it harmful?

Close relatives are not allowed to marry because their children are unhealthy, and children born with AIDS are also unhealthy. Why can people with AIDS conceal and marry and have children?
